To have your taxes prepared and filed at a VITA site, please bring:

• Photo identification
• Social Security numbers for you, your spouse and all dependents
• Birth dates for you, your spouse and all dependents
• Interest and dividend statements (Forms 1099), if applicable
• Bank Routing and Account numbers for Direct Deposit of your refund (voided check or bank statement)
• Total amount paid for day care, provider’s name and tax identifying number, if applicable

If married filing joint tax return, your spouse must be present to sign the required forms.
